Assam - Add or Delete Name in Ration Card

Procedure

  1. Apply In-Person

  2. Ration Card holder ought to approach the Food Civil Supplies& client Affairs authority of the District/Sub-Division(C) or ration workplace to feature or delete name in card.

  3. Applicant shall acquire the shape from the individual workplace (please procure the shape if required) Or a written/electronically written application on A4 plain sheet as suggested shall be used.

  4. Please submit the finished application together with the desired documents to the individual authority.

  8. The mortal is hip through SMS at numerous stages i.e., Advance intimation concerning date of visit of the official to his place of residence for verification of his application, Approval/Rejection of his application.

  9. After ground level verification card are going to be issued in two – three weeks as notified by the department

  6. Proof of Date of Birth (Birth Certificate/  X Pass Certificate  /declared/other)

  7. Surrender Certificate in original (for inclusion, if any)

  8. Death Certificate just in case of death of a member so as to delete

Document needed for Adding member on account of wedding

  1. Marriage Certificate

  2. Original card with husband’s name

  3. Name Deletion Certificate from the oldsters of Bride

  4. Document needed for Adding New Born kid in card (above five years)

  5. Birth Certificate issued by the native municipal body or competitive authority

  6. Original card of the family with parent’s name

  7. ID Proof of fogeys like PAN Card, Aadhaar Card

Eligibility

  1. A valid card holder.

  2. The mortal and his/her family should be Indian voters and may be resident of the state.

  3. The mortal or his members of the family should not possess any family card in any State in Republic of India (If that they had one, it's to be withdrawn).

  4. The mortal and members of the family should be shut relatives.

  5. New born higher than five years

  6. Addition of member name once wedding

Fees

Fee info is obtained directly from the department workplace whereas applying.

Validity

Validity remains same

Usually Family positive identification is valid for time period and can be revived in five years or supported announcement.
