Punjab CM Amarinder Singh has decided to bring the total number of 9.5 lakh farmers and their family members under Ayushman Bharat Sarbat Sehat Bima Yojana in 2020-21. Punjab CM Amarinder Singh said beneficiaries could go to any of the 546 private hospitals and 208 public hospitals to receive treatment for up to Rs 5 lakh of 1,396 diseases including the serious disease treatments such as cancer surgery, heart surgery, rehabilitation cases and accidents under the health insurance scheme. government.

Farmers who wish to receive benefits according to the program can obtain an identification form from the relevant Market Committee office or commission company (Artijah) or can also be downloaded from the site www.mandiboard.nic. in the middle. Upon receipt of applications from farmers, the loading will be done in a specially designed section with the intention of subsequent eligible farmers being issued with 'Sehat Bima Cards'.

Procedure to Apply Online Punjab Sarbat Sehat Bima Yojana Application Form 2020:

All farmers who received a J-Form and those with ‘Sugarcane Weight Slips’, were eligible for the benefit under this scheme.

Eligible farmers were required to submit their applications and the necessary documents to their Market Committees or to the commissioners of the Commission (Arhtiya) on July 24.

Farmer insurance premium will be paid by the Punjab Mandi Board and the farming community would not have to pay.

In the Punjab State, more than 43 Lakh families are eligible for the scheme. These include:

  • Farmers in charge of J-Form,

  • Small retailers registered with the Customs and Excise Department as well

  • SECC 2011 data families,

  • Families of Smart Ration Card Holders,

  • Construction workers registered with Punjab Building and Other Construction Workers Welfare Board.

Note: This program will ensure the benefit of non-profit medical treatment up to Rs 5 lakh for 1,396 various diseases. Covered diseases include heart surgery, cancer treatment, joint replacement procedures, and treatment in emergency situations.

Procedure to Check Sehat Bima Yojana Beneficiary List:

Step 1- Visit the Official site of the Government Health Agency, Punjab namely sha.punjab.gov.in.

Step 2- On the homepage, you should click on Check Eligibility Under AB-SSBY

Step 3- Search for coverage under the page of AB-Sarbat Sehat Bima Yojna (AB-SSBY) will be displayed on the screen.

Step 4- On the web page, you will find 2 options to check your eligibility namely-

  • Scorecard number / PAN number / Registration ID / Farmer or Journalist ID according to search criteria

  • Search by word

Step 5- Click on the option you want.

Step 6- Enter the requested information

Step 7- Click on the status

Beneficiary Search Criteria: 

  • Ration Card Person: If your ration card looks like 'FAZ1234567' and change the FAZ to 03000 and search for '30001234567 ', if it is similar to MNS9809898 then enter MNS along with 03000 and search with -030009809898. If your ration card is old, contact your local depot manager for the new smart ration card number.

  • Farmer (j_form holder) The search ID will be the first 3 characters of the District Name + the last 6 numbers of Aadhaar + the first four letters of your name. For example: if your name is Mohan from Hoshiarpur District with the last six digits of Aadhaar as 123456, the search id will be the same as HOS123456MOHA

  • The Construction Worker Search ID will be the registration id issued by BOCWB, Punjab State.

  • The Small Business Search ID will be a PAN card number

  • Yellow card and Journalist Search ID which will be the first third character of the regional name + the last 6 numbers of Aadhaar with a link after the three digits + the first four letters of your name. For example: if your name is Harwinder from Jalandhar district and the last 6 numbers of Aadhaar such as 123456 with a hyphen (-) in the middle, the search ID can be: JAL123-456HARW

Procedure to Search Common Service Centre:

Step 1- Visit the Official Website of the Government Health Agency, Punjab namely sha.punjab.gov.in.

Step 2- On the homepage, you have to click on the citizen's corner.

Step 3- After that, you should click on the Common Service Center

Step 4- Now, you have to select your region

Step 5- After that, you have to click on the search

Step 6- Center details of CSC will be displayed on screen.

Step 2- On the homepage, automatically click on the page of the Hospital tab

Step 3- Now you have to click on the included hospitals

Step 4- After that, you should select a public or private, regional and special category

Step 5- Now you have to click on the search

Step 6- The list of the unlisted hospitals will be displayed on the screen.

Search De-Empanelled Hospitals:

Step 1- Visit the Official site of the Government Health Agency, Punjab namely sha.punjab.gov.in.

Step 2- On the homepage, automatically click the page on the Hospital tab

Step 3- Now you have to click on De-Empanelled hospitals

Step 4- As soon as you click on this link of a list of qualified hospitals will be displayed on the screen.

Download Referral Slip Format:

Step 1- Visit the Official site of the Government Health Agency, Punjab namely sha.punjab.gov.in.

Step 2- On the homepage, you should click on the hospital tab

Step 3- Now you have to click on Download Referral Slip Format

  • Small Traders - 0.46 lakh families under the Department of Property Tax, Punjab Government

  • J-Form farmers- 4.94 lakh J-Form farmers in charge of the Punjab Mandi Board, Punjab Government

  • Small and Medium Farmers- Smallholder Farmers of about 2.76 lakh and Marginal-Farmers of Punjab Mandi Board, Punjab State

Achievements of Sarbat Sehat Bima Yojana:

  • The Department of Health has issued 45 lakh health cards under Sarbat Sehat Bima Yojana and 4.66 lakh beneficiaries received the facility.

  • Under Sarbat Sehat Bima Yojana there are 767 active hospitals.

  • So far more than 6600 heart surgeries have been performed, 3900 have been performed jointly, 9000 cancer treatments have been performed and 96000 dialyses have been performed.

  • The number of health packages has been increased from 1393 to 1579.

  • There are 1396 packages for free secondary care and tertiary care in public and private hospitals in Punjab and Chandigarh.

  • This includes the treatment of serious illnesses.124 packages reserved for public hospitals

  • A three-day pre-hospital treatment package and the cost of a 15-day hospital stay are available.

  • The program was launched by the State government on Rajiv Gandhi's birthday on August 20, 2019.
